Cisplatin and DTIC  chemotherapy order

l         Start C/T protocol if data (BUN Cr, GOT, Na, K, Cl, CBC, DC, Platelet, EKG) O.K.

l         On diet as tolerable.

l         Record vital signs q4h, record I/O q8h, body weight QD.

l         Set up IV line on day 1 with

#1       D5S 500 cc.

#2       D5S 500 cc.

#3       D5S 500 cc.                   each run 500 cc./hr.

#4       Cisplatin 100 mg/m2, total _____ mg in 500 cc. N/S IV drip in 1 hr.

#5        20% mannitol 300 c.c. IV drip in 30 min.

#6       N/S 500 c.c.            run 500 cc./hr.

then  L-R + 1 Amp Vit C + 10 meq KCl   alternatively run 125 c.c./hr.

l         Day 2 - 5 IV fluid keep with

#1, #3, #5 D5S + 2 c.c. B-complex

#2, #4, #6 L-R + 1 Amp Vit C + 10 meq KCl     alternatively run 125 c.c./hr.

l         DTIC 200 mg/m2 , total _____ mg, in D5S 500 cc IVF run 125 cc/hr on D1, D2, D3.

l         Antiemetics with

□ #2 100 cc,        #3 250 cc. left for antiemetic solvent

1.         Zofran 8 mg in 100 cc D5S (#2) IV drip in 10 min.

2.         Dexan 10 mg in 100 cc. D5S (#3) IV drip in 10 min.

(1),(2). at 30 min. before cisplatin begins and repeat q8h x 3 doses.

l         Akineton 1 Amp IM prn if EPS attack.

l         Novamin 1 Amp IM q4h prn.

l         Dexamethasone 10 mg iv prn, then q4h x 3 doses.

l         Check BUN Cr, GOT, Na, K, Cl, CBC, DC, Platelet QD.

l         Please notified Dr. if urine < 200cc/8hrs, high fever, conscious change, or dyspnea.

l         Oral medications:

(a)Primperan 3# qid.

(b)Ativan 1# qid prn nausea or emesis.

(c)Zofran (4mg) 2# q8h.

(d)Dexan 2# bid x 3 days, 1# bid x 2days.

l         Antiemetics with

□ #2 100 cc,        #3 250 cc. left for antiemetic solvent

1..Zofran 8 mg in 100 cc D5S (#2) IV drip in 10 min.

2.Dexan 10 mg in 100 cc. D5S (#3) IV drip in 10 min.

(1),(2). at 30 min. before cisplatin begins and repeat q8h x 3 doses.

Oral medications:

Zofran (4mg) 2# q8h.

Dexan 2# bid x 3 days, 1# bid x 2days.

Or

1.Primperan (2, 3) mg/kg, total _____mg in #3 250 cc D5S IV drip in 10 min .

2.Vena 50 mg in 100 cc. D5S (#1) IV drip in 10 min.

3. . Dexan 20 mg in 100 cc. D5S (#2) IV drip in 10 min.

(1).(2).(3). at 30 min. before cisplatin begins and repeat

(1). at 1.5, 3.5 hours after cisplatin begin (#6 and half bottle).
