1、 Check 24hrs CCR on D1。On diet as tolerable.

2、 Record vital signs q4h, record I/O q8h, body weight QD.

3、 Check BUN Cr, GOT, Na, K, Cl, CBC, DC, Platelet, EKG.

        Start C/T protocol  if data OK, approved by attending or oncology fellow 。

4、Set up IV line with D5S 500 ml run 125 ml/h X2

5、Give premedication with

a)     Cimetidine 300 mg IVF。

b)     Vexan 50 mg IVF。

c)      Dexan 20 mg IVF。

     a)b) at 3 0 min before Paclitaxel begins。

c) at 12 and 6 hrs before Paclitaxel

6、Paclitaxel ( 135 – 175) mg/m² total    gm in 300ml D5w run 100ml/h with     

   0.22um in -line filter after  IV infusion。

7、IV line with D5S 500 c.c.  keep open。

8、Carboplatin (200, 300, 400) mg/m2,  total______mg in 500 c.c. N/S run 125 c.c./hr, one hour after taxol.

9、a)      Tropisetron 1pc in 100 cc D5S IV drip in 10 min before carboplatin.

b)     Vean 30 mg in 100 c.c. D5W IV drip in 10 min。

c)      Dexan 20 mg in 100 c.c. D5W IV drip in 10 min。

   a)b)c) at 30 min before starting Carboplatin,  and at the end of Carboplatin

         infusion, each。

10、Novamin I Amp IM q4h pm if nausea , vomiting。

11、Akineton 1Amp IM prn if EPS attack。

12、 Record 1/0 q8h。

13、Discharge order：

   a) Primperan 2# qid。

b) Ativan 1#qid。

c) Dexan 2# bid x 3 days，than 1# bid x 2 days。
